Getting started with your treatment

ATTENTION: If you are new to Biscuit — Please book an orientation appointment via the Biscuit Health app prior to administering any treatment. Your health coach will walk you through how to administer treatment effectively & answer any questions you may have.

NEVER BEGIN TREATMENT PRIOR TO YOUR ORIENTATION APPOINTMENT.

If you experience an emergency, inject your Epinephrine Pen (EpiPen) into thigh (if you have one) and dial 9-1-1 prior to contacting Biscuit Health.

If you experience a minor side effect, please follow our Action Plan below.

Welcome to Biscuit! We are excited to help you tackle your allergies. Please ensure you have received the following in your kit:

Your clinician may also prescribe you an Epinephrine Injection Pen and Practice Device if you've indicated you have Asthma and score lower than 23 on our Asthma Control Test.

If you are missing any items, please reach out to a care coach via the Biscuit Health app as soon as possible.


How To Administer Your Drops

  1. Prime the pump by pressing down firmly and quickly a few times until some solution comes out. The pumps are created to administer a metered dose. The metered dose will dispense when the pump is pressed all the way down (you will meet some resistance at the end). If you experience any issues with your pumps, please reach out to the care team via the Biscuit Health mobile app.

  2. Place the pump under your tongue and firmly press down the pump to administer a drop. 1 pump = 1 drop.

  3. Hold the drop under your tongue for 2 minutes and then swallow.


Dosage & Phases

Overview:

You'll go through 2 phases of treatment throughout your journey; build-up & maintenance.

  1. Build Up — The purpose of the build-up phase is to introduce your body to immunotherapy while reducing potential minor side effects. The build-up phase lasts 30 days and you will need your GREEN + YELLOW + RED bottles for this phase.

  2. Maintenance — This is the bulk of your immunotherapy treatment. After 30-days, you will need to consistently use the RED bottles to continue your treatment until the end date determined by your clinician (usually 1-2 years). It is imperative you minimize missed dosages, as extended periods of misses doses may require you to start again with another build-up, slowing down your treatment and reducing efficacy.

Day 1-30: Build-Up Phase

This phase will last 30 days, and you will need your GREEN, YELLOW & RED bottles. Hold onto your empty bottles for recycling, your next shipment will include a return label.

Day 31+: Maintenance Phase

Congrats! You've finished your build-up phase, and are ready to begin the full strength treatment for your allergies. Remember it's imperative you minimize missed dosages, as extended periods of misses doses may require you to start again with another build-up, slowing down your treatment and reducing efficacy.

Action Plan — Side Effects & Reactions

IN CASE OF EMERGENCY: Dial 9-1-1 immediately prior to contacting Biscuit Health.

Minor Side Effects or Reactions:

Minor side effects may occur during your build-up phase and are normal; they typically occur within minutes or hours of administering your drops. On average, a third of patients taking sublingual immunotherapy may experience minor side effects.

Mild to Moderate Local Reactions

Action Plan:

Mouth: Bothersome itching in the mouth and/or mild swelling of the lips and/or tongue
Throat: Bothersome itching, irritation, and/or mild tightness in the throat
Ear: Bothersome itching in the ears
Gastrointestinal: Mild/persistent abdominal pain, nausea, and/or cramps

  1. Take an over-the-counter antihistamine such as: Claritin, Allegra, Aerius, Benadryl, Zyrtec, or Xyzal

  2. If symptoms improve within 60-minutes, no further action is required.

  3. If symptoms do not improve, or worsen, notify your care coach by sending us a message in the Biscuit Health App.

Major Reactions:

IN CASE OF EMERGENCY: Inject Epinephrine pen and dial 9-1-1 immediately prior to contacting Biscuit Health.

Sublingual immunotherapy very rarely causes life-threatening allergic reactions, such as anaphylaxis, however, below is some guidance to identify and respond to a severe reaction.

Severe Local Reactions

Action Plan

Mouth/Throat: Swelling that causes voice to sound hoarse and/or throat closing

  1. Inject Epinephrine Pen (EpiPen) into thigh — UNDER 30 kg: 0.15mg / OVER 30 kg: 0.3mg

  2. Call 9-1-1 and inform them you are experiencing an anaphylactic event.

  3. If symptoms do not improve, or if symptoms return, administer a 2nd dose of epinephrine at least 5 minutes AFTER the previous dose.

  4. Notify your care coach by sending us a message in the Biscuit Health App.

Severe Systemic Reactions

Action Plan

Skin: Hives and/or swelling and/or redness all over the body
Lungs: Repetitive cough, shortness of breath, and/or wheezing
Heart: Dizziness, weak pulse, and/or passing out
Gastrointestinal: Severe abdominal pain, cramping, vomiting, and/or diarrhea

  1. Inject Epinephrine Pen (EpiPen) into thigh — UNDER 30 kg: 0.15mg / OVER 30 kg: 0.3mg

  2. Call 9-1-1 and inform them you are experiencing an anaphylactic event.

  3. If symptoms do not improve, or if symptoms return, administer a 2nd dose of epinephrine at least 5 minutes AFTER the previous dose.

  4. Notify your care coach by sending us a message in the Biscuit Health App.

*Once treated and resolved, report any reactions to your Biscuit clinician via the Biscuit Health App.
